What is the PSI?

The Individualised Services Plan (PSI) is a health-care programme that targets people with severe mental disorder (SMD) who have difficulty using the mental health services. |

Who does it coordinate with?
With the professionals and services of the local Public Mental Health Network (CSM, SRC, hospitalisation), Health Services, Social Services, Legal Services and community teams involved in their care-provision process.
Who does the PSI provide care to?
The PSI provides care to residents of the districts of Les Corts and Sarrià-Sant Gervasi in Barcelona aged over 18 with severe mental disorder (SMD) with a major family and social repercussions and/or inadequate use of services.
How to request a consultation?
Through referral by the psychiatric, health or social network professional who is aware of the patient's situation.
The referring professional must send the referral sheet to the programme and contact the PSI team.
Who makes up the health-care team?
It is a team of professionals comprised of trained nurses and/or social workers and/or social educators who perform the function of individualised project coordinators (CPI), and two psychiatrists from the CHMLC responsible for coordinating the programme.
When is it open?
The activities are carried out in mornings and afternoons from Monday to Friday.
